The next dialog to appear gives you a list of all the questions you have asked. At present this list is empty, so click Add to start adding questions. You will see this list five more times, and each time your questions will be listed.

The dialog below then appears - select 'personal information'. You can change the phrasing of the question in the bottom half of the dialog box. Click Next to continue.
