Type in the labels for the choices you are offering the user in the upper part of the dialog box.
I selected a drop-down menu to present this question. This is good for long lists of alternatives, the list does not occupy vertical space on the page.
You will need to choose a name for the information, I chose 'employment'. Then click Next when done.

You should have returned to the dialog which lists the questions - just click Add and then select Paragraph and alter the prompt text as shown below. Then click Next.
