Supply a name for your paragraph - I used 'BestThing' - then click Next to return to the Add question dialog.
Repeat the last two steps to insert the 'WorstThing' paragraph.

The question is the overall rating for the Web site. Assuming you are back at the Add questions dialog, click Add to choose the next (and final) question.
Select 'One of several options' from the list of question types, add the prompt text then click Next.
