Select which kinds of contact information you want from the samples below. You might want to mark some items as 'optional' when the page is finished, people are a bit suspicious about giving phone numbers on the Web.
The 'base variable' tells you what the name of these items will be when the form is returned to you by e-mail. Click Next to move on.

You return to the Add item dialog box. Click Add to continue.
Select 'One of several options' and alter the prompt text to make the Occupation question, then click Next.
