Add the rating headings (Excellent, Good and so on) to the upper part of the dialog. I have used a 'radio button' for this item. Provide a name for the information in the lower part, then click Next.

You return to the Add question dialog - just click Next this time, no more questions!
You will see the presentation dialog below - just choose your preferences and select Next.
